View completion status using Patient List in Smart Capture

If activated for your programme and your user role, Smart Capture allows you to view the completion status of patients by different survey rounds. For patients who have been invited to complete a survey, or started but not yet completed, you can prompt completion while they are in your care setting.

Below are steps on how to find your patient list, and how to find a patients unique survey link/QR code for a particular survey round.

Where to find your Patient List

1. Expand your apps button in your top navigation panel and navigate to 'Smart Capture'.

2. Click 'Patient List'


Patient List Overview

Patient Info

Details include:

  • Unique ID such as 'Patient ID'

  • Last Name, First Name

  • Date of Birth

Search Bar

As only the 46 patients most recent responses are listed in the Patient List, you can use the search bar to find a specific patient you are looking for.

Completion Statuses

Invited

A record appears as Invited in Yellow once an email or SMS invitation has been sent to the patient.

Click to bring up a QR code (like the example below) so the patient can complete the survey

Started

A record appears as Started in Yellow once a response that has been sent via email or SMS has been opened by the patient (but not completed).

Click to bring up a QR code so the patient can resume the survey.

Completed

A record appears as Completed in Green once an SMS or Email survey has been completed, or after a collected record has matched with a record.

Click to view the full response summary for that specific survey.

Collected

A record appears as Collected in Blue once a QR code Pre-op response has been completed by a Patient
