Clinicians View - Individual Patient Responses

With clinicians view you can see your individual patient’s responses across survey rounds, providing in-depth insight into the patient’s self-reported journey and outcomes.

Note that access to this chart will be based on your permissions and programme.

Main Features

To see a list of all patients, just click on the Search button. Otherwise, type the patient’s details to find the relevant patient information.

Select the patient you want to see the journey for. This will show you their scores across all completed survey rounds for all outcome measures.

The relevant survey round is found on the bottom x-axis. Refer to a trend line’s appropriate legend colour to see the outcomes measure, or hover over the line. This will also tell you the outcome measures score for the relevant survey round.

If an outcomes measure is in the green area, then their outcomes are improving, if moving into the red then this indicates that it’s getting worse.

You can select which outcome measures you want to report on in the top right menu.

Below the chart, you can see how the patient answered the survey for each outcomes measure across survey rounds. The colour indicates the severity of the patient’s outcomes. Additionally, you can click view for ‘Full response’ to see the full completed survey for the relevant survey round.
